MRC SUMMER REGISTRATION 2008

This registration and release form is for MRC (Marengo Recreation Commission) summer activities.  All boys and girls K-6th grade who are interested in playing baseball or softball need to complete both sides of the bottom of this form and bring it with them to the meeting / registration at the high school on Sunday March 9 at 5:30 in the high school Gymnasium.

      RELEASE FORM ON BACK MUST ALSO BE SIGNED!  Divisions are as follows:

 

K-1                Boys & Girls T-Ball

2-3                Boys-Pee Wee, Girls-Power Puff

4-6       Boys & Girls Little League

Registration Costs:  $35 – one child

                                       $65 – two children

                                     $90 – three children

                                     $25 – per child for each additional child after three

PLEASE NOTE:    - There is a 2 week period for late sign ups, until March 23rd, at that time there will be no one accepted, unless they are           new to town.  If they hit this late period there will be a $20 late sign up fee.

   - 1ST Practices will not start until April 1st. Do not look for the K-3rd to start until the 1st or 2nd week of May.

   

Parents of children who plan to participate In MRC activities must attend this meeting!

There will be a short meeting before Registration.  This meeting will cover the expectations of the parents and kids in all sports sponsored by the MRC.  All parents will be expected to volunteer as a coach or work in the concession stand.  The registration fee of $35.00 will be due at this time. This will include one free ticket per registration to the Cedar Rapids Kernels Game on Friday June 6th, MARENGO NIGHT. If your child isn't registered and dues paid at this meeting/registration, an additional $10.00 late fee will be charged.  There is an additional charge for ordering uniforms after original order is placed and it causes a delay in receiving uniforms.

 

These are your kids programs.  Get involved!

A board has been formed to run the Marengo Recreation Commission. Anyone is welcome to attend these meetings. Contact Ray Cooling if you are interested.  Also, we are in need of volunteers to be coaches, assistants, and to help with other tasks to make this summer run smoothly.  You will be asked to sign up for working in the concession stand before you leave.

RELEASE

 

The undersigned                                            and/or                                              mother/father or guardian, each being of legal age, do hereby acknowledge that our son/daughter                                           will be participating in athletic programs established and supervised by the Marengo Recreation Commission.  We do hereby release, acquit and forever discharge said Marengo Recreation Commission and the city of Marengo, Iowa, and any related persons, employees and agents, from all liability whatsoever, including all claims, demands and causes of action of every nature affecting us or either of us jointly or severally, or on behalf of our minor child's participation in athletic programs offered by the Marengo Recreation Commission.  We further agree that this release covers all injuries and damages, whether known or not known, and which may hereafter appear or develop arising from the participation referred to above.

 

That we have each read the foregoing release, and understand its terms and freely and voluntarily sign the same.  That we execute this release solely in reliance upon our own knowledge, belief and judgment, and not upon any representations made by the released parties or others in their behalf.
